What is the BP Success Zone Program membership card?
The BP Success Zone Program membership card is the key to taking advantage of the Program offers. It is a savings card found inside the BP Success Program Zone Program kit that is given to patients by their doctor. Patients will call the number on the membership card to enroll in the Program to receive the full benefits.

How do patients get a membership card if they don't already have one?
Talk to your patients about the BP Success Zone Program. If you prescribe DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets to treat your hypertensive patients, be sure to give them a BP Success Zone Program kit, containing a membership card.

How does the BP Success Zone Program membership card work?
Patients should follow these steps to use their $10 savings* offer:

Join the Program online or by calling 1-866-630-0350. |
 |
Take the prescription and their membership card to a pharmacy, giving them both to the pharmacist. Up to the $10 amount* will automatically be deducted from their cash or copayment amount. |
 |
Patients should keep their receipts with the prescription drug name and out-of-pocket costs (amount not covered by insurance which they have paid) for other offers. |

How do patients enroll in the BP Success Zone Program?

My patient filled their medications but forgot to present their membership card. Can they get reimbursed?
The next time your patient goes to the pharmacy, he or she should take their membership card. Have your patient present it to the pharmacist at the next refill of DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets to receive the savings.

My patient was entitled to a $10 savings but did not receive the full $10 savings. What should they do?
If the copay is less than $10*, please note that the savings covers up to $10 toward the copay on the medication or the full copay amount, whichever is less. If patients have a copay of less than $10, the savings would cover the copay completely, but the difference between the $10 savings and the copay cannot be paid out in cash or applied to a future purchase.
If the copay is more than $10 and your patient did not receive the full $10 savings, they can call 1-866-968-7270 to speak to a representative who can assist them.

My patient was in the now-expired Take Action for Healthy BP Program. How do they get a BP Success Zone Program membership card?
If your patient was a Take Action for Healthy BP Program member, instruct your patient to ask for a BP Success Zone Program kit or to join the Program online. Once they have a kit, they can enroll in the Program or they can enroll online. Your patient will be eligible for all the benefits of the Program including $10 off* their next fill or refill of DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets, ongoing support materials and all the online tools and features.
