Patients can receive from Novartis reimbursement for up to four months of out-of-pocket drug costs if, after taking the maximum dose of either DIOVAN HCT® (valsartan/hydrochlorothiazide 320/25 mg) tablets, EXFORGE® (amlodipine and valsartan 10/320mg) tablets or TEKTURNA® (aliskiren 300 mg) tablets for at least 30 days, their blood pressure is not controlled to the goal determined by their healthcare professional.*

How does the BP Goal Money-Back Guarantee work?
To be eligible for the guarantee, patients must have joined the BP Success Zone Program by activating their membership card. Patients must produce copies of the original receipts for up to 4 months of treatment on DIOVAN, DIOVAN HCT, EXFORGE or TEKTURNA. The patient must also provide a Guarantee Affirmation Form, signed by his or her healthcare professional, stating that the patient was not able to reach the blood pressure goal set by his or her healthcare professional.
A Guarantee Affirmation Form can be downloaded in the Members' Area of www.bpsuccesszone.com. Patients can also request an Affirmation Form by mail or call 1-866-968-7270.

Complete and sign the Guarantee Affirmation Form for your patient.
Have your patient mail the Guarantee Affirmation Form, as well as their original prescription receipt showing the prescription drug name and out-of-pocket cost for their DIOVAN, DIOVAN HCT, EXFORGE or TEKTURNA prescription as proof of purchase to:

Your patient should do this no later than 30 days from date of completion of the Guarantee Affirmation Form. No photocopies are accepted. Have your patient retain a copy for their records.
Your patient will receive a complete refund for up to 4 months of their out-of-pocket prescription costs (costs that are not reimbursed by insurance) for DIOVAN HCT® (valsartan/hydrochlorothiazide 320/25 mg) tablets, EXFORGE® (amlodipine and valsartan 10/320mg) tablets or TEKTURNA® (aliskiren 300 mg) tablets. How do I report a patient's blood pressure numbers for the guarantee?
You don't need to send your patient's blood pressure numbers for the guarantee. You simply determine the target blood pressure goal for your patient before the treatment. If they have not reached that goal after being on the highest dose of DIOVAN® (valsartan 320 mg) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide 320/25 mg) tablets, EXFORGE® (amlodipine and valsartan 10/320mg) tablets or TEKTURNA® (aliskiren 300 mg) tablets for at least 30 days:

How do patients get a refund on the BP Goal Money-Back Guarantee?

Why hasn't my patient received a BP Goal Money-Back Guarantee rebate in the mail?
Patients should allow up to 8 weeks for the BP Goal Money-Back Guarantee rebate check to be delivered in the mail.
If it has already been 8 weeks or more and patients have not received reimbursement for any portion of their prescription drug costs as part of any government assistance or medical insurance and they do not live in a country outside the U.S. and Puerto Rico, please advise them to call 1-866-968-7270.
If a patient has been reimbursed for a portion of their prescription drug costs as part of any federal government related healthcare programs, including Medicare, Transitional Assistance, Medicaid, TRICARE, VA (Veterans Administration), State Maternal, Child Health Block Grants, they are not eligible for this offer due to state and federal regulations.

If a patient sends in the guarantee, can they still use the BP Success Zone Program membership card?
If patients have not done so already, they may use the $10 membership card if they are still taking DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets. If patients discontinue taking DIOVAN, DIOVAN HCT, EXFORGE or TEKTURNA, they will not be able to use the membership card on another medication. Patients should hold on to their membership number, as it will let them take advantage of other benefits of the BP Success Zone Program.

How can patients get a Guarantee Affirmation Statement for the money-back guarantee?

Are physicians required to provide their DEA# on the Guarantee Affirmation Statement?
Yes. In order to prevent fraud, we need a DEA# to ensure patients goals are being set by physicians.

Once a patient sends in the Guarantee Affirmation Statement, can he or she still take advantage of the Omron® Blood Pressure Monitor rebate?
If the patient has not already taken advantage of the rebate, he or she can submit a rebate for the Omron® Blood Pressure Monitor. Patients need to be sure to save a copy of their DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets receipts, as receipts are needed to redeem the rebate. Patients should hold on to their membership number, as it will allow them to take advantage of other benefits of the BP Success Zone Program.

If a patient redeemed the Take Action for Healthy BP Program money-back guarantee, can they take advantage of the BP Goal Money-Back Guarantee?
We're sorry, but there is a limit of one offer per person on the BP Goal Money-Back Guarantee. Patients who submitted claims for the now-expired Take Action for Healthy BP Program money-back guarantee cannot submit a claim for the BP Goal Money-Back Guarantee in the BP Success Zone Program as well.
