What is the Omron® Blood Pressure Monitor offer?
When patients enroll in the BP Success Zone Program, they will receive a membership card. This membership card will allow them to receive a rebate on the purchase of an Omron® manufactured blood pressure monitor up to a $40 value.*
They can also choose any Omron® Blood Pressure Monitor or blood pressure monitor that is sold by Kroger, Eckerd, or Wal-Mart's ReliOn® brand, which is marked "Manufactured by Omron®" on the box. Novartis will reimburse your patient up to $40 toward the cost of any such blood pressure monitor.
In Minnesota and Massachusetts, the Program offers are slightly different. Please click here for details.
This Program is only available to residents of the U.S. and Puerto Rico.

How does the Omron® Blood Pressure Monitor offer work?
Patients should allow 6-8 weeks for delivery of rebate check(s). Offer is limited to one Omron® Blood Pressure Monitor rebate check per person.
This offer expires on June 30, 2009. It is not valid for patients reimbursed under Medicaid or similar federal or state programs. The offer is void where prohibited by law, taxed or restricted. Novartis Pharmaceuticals Corporation reserves the right to rescind, revoke or amend this offer without notice.

At this time, Novartis does not have a process to acknowledge that paperwork has been received. However, if it has been more than 8 weeks since your patient submitted their claim and they have not yet received their check, your patient can call 1-866-968-7270 and we will investigate their claim.

My patient received a rebate for an Omron® Blood Pressure Monitor through the Take Action for Healthy BP Program money-back guarantee. Can my patient receive another blood pressure monitor in the BP Success Zone Program?
We're sorry, but there is a limit of one Omron® Blood Pressure Monitor rebate per person. Patients who submitted rebate claims under the Take Action for Healthy BP Program cannot submit a claim for another rebate through the BP Success Zone Program.

We apologize, but as of October 1, 2005, the offers of the former Take Action for Healthy BP Program are no longer valid. However, the Omron® Blood Pressure Monitor rebate is still available under the BP Success Zone Program if your patient did not redeem this offer through the Take Action for Healthy BP Program.

My patient was a member of the Take Action for Healthy BP Program. Does my patient need to use the Healthy BP card when submitting an Omron® Blood Pressure Monitor claim?
No. If patients were members of the Take Action for Healthy BP Program and enrolled in the new BP Success Zone Program, they do not need to use their old card number. They should simply use their new BP Success Zone Program membership card/Member ID number when submitting a claim.
