What does the membership card cover?
The membership card will provide up to 14 days of medication. It entitles patients to up to 14 DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets at the dosage prescribed by their healthcare professional.

How does the membership card work for patients who live in Massachusetts or Minnesota?
Up to a 14-day free sample* (max. 14 pills) on a fill or refill of DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets is available to consumers who enroll in the BP Success Zone Program. To receive these savings they should get a prescription for a Novartis high blood pressure medication and a BP Success Zone Program kit from their healthcare professional, then enroll in the Program by calling 1-866-630-0350 or online at www.BPSuccessZone.com. Each person will receive a BP Success Zone Program membership card. Patients should take the membership card and a prescription for up to a 14-day supply (max. 14 pills) to the pharmacy. The pharmacist will redeem this membership card, and the purchase will be discounted at the register.

How many membership cards can patients use?
A person can redeem only one membership card.

Can patients make extra copies of the membership card?
No, patients cannot make extra copies of the membership card. Each membership card has a unique ID number on it used for the pharmacy redemption. This ID number will only be accepted at the pharmacy once. The pharmacist will receive a reject message when trying to submit a membership card with an identification number that has already been used.

Where can patients redeem the membership card?
Any retail pharmacy that transmits claims online can accept the membership card for DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets. Pharmacies that do not transmit claims online are welcome to submit Universal Claim Forms. Dendrite, the company that provides the membership cards, welcomes all pharmacies to transmit the membership cards and there are no contractual requirements. For pharmacy information on how to transmit claims to Dendrite, please call 1-866-968-7270. Live operator support is available from 8:00 a.m. to 5:30 p.m. EST, Monday through Friday. During the hours of 5:30 p.m. to 9:00 p.m. EST, Monday through Friday and on Saturdays from 9:00 a.m. to 3:00 p.m. EST we offer after hours support with on call agents who will return your call within a 15-minute window.

Do patients have to pay a copayment?
No. The membership card entitles them to up to a 14-day free sample of DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets (max. 14 pills).

Do patients need a prescription?
Yes. In order to redeem the membership card, patients need a valid signed prescription from their healthcare professional for up to 14 days of DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets (max. 14 pills). As their healthcare professional, you have to sign the membership card and indicate the correct dosage amount for it to be valid. The prescription and membership card must be presented at the pharmacy to receive the free sample.

How long do patients have to redeem the membership card?
The membership card is valid through June 30, 2009. Please refer to the section of the membership card that notes the expiration date.

Are all pharmacies participating in this Program?
All pharmacies are invited to process the membership card for DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets. There are no network requirements. If a pharmacy refuses to accept a membership card for any reason, patients should simply request that their prescription and membership card be returned so that they can take the membership card to another neighborhood pharmacy.

If a patient filled their prescription at the pharmacy, but forgot to provide the membership card, how can they get reimbursed?
The membership card can be used once for any future refill. They should simply take the membership card with them to the pharmacy with a 14-day prescription (max. 14 pills) for DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets and present it to the pharmacist. They will receive the 14-day free sample (max. 14 pills) at that time.

My patient received refill benefits as a member of the now-expired Take Action for Healthy BP Program. Are they entitled to the 14-day free sample as well?
Yes. By joining the BP Success Zone Program, your patient is entitled to the 14-day free sample (max. 14 pills) regardless of whether or not they received benefits in the former Take Action for Healthy BP Program.
