How does the BP Goal Money-Back Guarantee work?
To be eligible for the guarantee, patients must have joined the BP Success Zone Program by activating their membership card. Patients must produce copies of the original receipts for up to 4 months of treatment on DIOVAN, DIOVAN HCT, EXFORGE or TEKTURNA. The patient must also provide a Guarantee Affirmation Form, signed by his or her healthcare professional, stating that the patient was not able to reach the blood pressure goal set by his or her healthcare professional.

Have your healthcare professional complete and sign the Guarantee Affirmation Form.
Mail the Guarantee Affirmation Form, as well as your original prescription receipts showing the prescription drug name and out-of-pocket costs for your DIOVAN, DIOVAN HCT, EXFORGE or TEKTURNA prescription as proof of purchase to:

How do I report my blood pressure numbers for the Money-Back Guarantee?
You don't need to send your blood pressure numbers for the Money-Back Guarantee. Your healthcare professional determines your target blood pressure goal before the treatment. If you have not reached that goal after being on the highest dose of DIOVAN HCT® (valsartan/hydrochlorothiazide 320/25 mg) tablets, EXFORGE® (amlodipine and valsartan 10/320mg) tablets or TEKTURNA® (aliskiren 300 mg) tablets for at least 30 days, follow these steps.

If you discontinue taking DIOVAN, DIOVAN HCT, EXFORGE or TEKTURNA, you will not be able to use the membership card on another medication.

If you have not done so already, you may submit your rebate for your Omron® Blood Pressure Monitor.** Be sure to save a copy of your DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets receipt, as it is needed for your rebate. You should still hold onto your membership number, as it will let you take advantage of other benefits of the BP Success Zone Program.

We're sorry, but there is a limit of one offer per person on the Money-Back Guarantee. Users who submitted claims for the now-expired Take Action for Healthy BP Program's Money-back Guarantee cannot submit a claim for the Money-Back Guarantee in the BP Success Zone Program, as well.
