	Activate your BP Success Zone Program membership card and fill your select Novartis high blood pressure medicine prescription. Be sure to use your membership card at the pharmacy when you fill your prescription.
	Purchase any Omron® Blood Pressure Monitor or blood pressure monitor that is made by Kroger, Eckerd, Wal-Mart's ReliOn® brand or McKesson's Sunmark brand, which is marked "Manufactured by Omron®" on the box.
	Within two weeks of purchase of your Omron® Blood Pressure Monitor, complete the rebate form in your BP Success Zone Program kit and return it with the following three items: Original receipt for the monitor Original UPC code (bar code) from the monitor package A photocopy of a pharmacy receipt showing the drug name of the Novartis high blood pressure medications and out-of-pocket cost of your high blood pressure medicine prescription. Remember to retain copies for your records.

When you fill your new prescription at the retail pharmacy, please follow these steps to use your savings offer:

	Get your prescription of DIOVAN® (valsartan) tablets, DIOVAN HCT® (valsartan/hydrochlorothiazide) tablets, EXFORGE® (amlodipine and valsartan) tablets or TEKTURNA® (aliskiren) tablets from your healthcare professional.
	Activate the membership card by calling 1-866-630-0350. You can also join the Program online, and an activated card will be mailed to you.
	Take your prescription and your activated membership card to your pharmacy, giving them both to the pharmacist. Up to $10* will automatically be deducted from your cash or copayment amount.

Remember to keep your receipts with the prescription drug name and out-of-pocket costs (amount not covered by insurance which you have paid) for other offers and Program benefits.

There are many models to choose from under $40, but if you prefer to buy a more expensive one, Novartis will reimburse you up to $40 toward the cost of any such blood pressure monitor.
