The BP Goal Money-Back Guarantee

Patients can receive from Novartis reimbursement for up to four months of out-of-pocket drug costs if, after taking the maximum dose of either DIOVAN HCT (320/25 mg), EXFORGE (10/320 mg), or TEKTURNA (300 mg) for at least 30 days, their blood pressure is not controlled to the goal determined by their healthcare professional.

To be eligible for the guarantee, patients must have joined the BP Success Zone Program by activating their membership card. Patients must produce copies of the original receipts for up to 4 months of treatment on DIOVAN, DIOVAN HCT, EXFORGE or TEKTURNA. The patient must also provide a Guarantee Affirmation Form, signed by his or her healthcare professional, stating that the patient was not able to reach the blood pressure goal set by his or her healthcare professional. A Guarantee Affirmation Form can be downloaded from the Members' area of www.BPSuccessZone.com. Or, to request a Guarantee Affirmation Form by mail, call 1-866-968-7270.

Free Omron® BP Monitor, With Rebate

	Purchase any Omron or Omron-manufactured BP monitor from your favorite retailer.
	Complete the rebate form in your BP Success Zone kit and submit it within two weeks of monitor purchase with a. original UPC code from the monitor packaging b. original store receipt for the monitor purchase c. a photocopy of a pharmacy receipt for your Novartis blood pressure medicine prescription
	Novartis will issue a rebate check for up to $40 toward the purchase price of the monitor (please allow 6-8 weeks).

*Free offer valid on Omron model HEM-432CN or other program-approved models equal to or less than $40.
